Expanding Exchange Brings Care Coordination to the Underserved
CHICAGO, ILLINOIS, January 23, 2014 — Five new community-based service providers have successfully connected to the Illinois Health Information Exchange (ILHIE). J2 worked closely with ILHIE and the Alliance of Chicago to onboard these stakeholders — Near North Health Service Corporation, Erie Family Health Center, Heartland Health Outreach, Howard Brown Health Center, and PCC Community Wellness Center — to the exchange.
Together, J2 and the Alliance helped each stakeholder through several critical onboarding tasks, including loading of their patient populations into ILHIE’s master patient index and integration with the exchange via IHE profiles such as PIX and XDS.b. J2 also worked with InterSystems to effect the necessary HealthShare configuration updates for each stakeholder.
