Care Coordination For the Mentally Ill, in Partnership With BHIX, Earns Share of Second Place
BROOKLYN, NEW YORK, January 29, 2014 — Healthcare Informatics has awarded Maimonides Medical Center with a share of second place in its 2014 Innovator Awards for its work delivering coordinated care services for seriously mentally ill (SMI) populations in New York. The project relies heavily on work done by J2 on behalf of the Brooklyn Health Information Exchange to allow Maimonides and its partners to share continuity of care documents via HealthShare.
