Two Recipients of CHART Grant to Leverage Exchange to Provide More Efficient and Effective Accountable Care
GREENFIELD and WARE, MASSACHUSETTS, January 9, 2014 — Baystate Franklin Medical Center in Greenfield and Baystate Mary Lane Hospital in Ware are among a group of Western Massachusetts community hospitals that received nearly $2 million in grants from the state’s Health Policy Commission as part of the Community Hospital Acceleration, Revitalization, and Transformation (CHART) program. The purpose of the grants is to help community hospitals provide higher quality care at a lower cost, reducing the need for patients to seek such care at larger health systems in Boston and elsewhere.
Both hospitals will rely on the Pioneer Valley Information Exchange (PVIX) for HIE capabilities that will be critical to the success of the program. Much of the responsibility for connecting these new stakeholders to the exchange will fall to J2.
